Alliance Health is responsible for ensuring that some of the most vulnerable members of the North Carolina Medicaid system have timely access to specialty care to address their overall health and well-being. Leveraging its long history as a behavioral health care organization, Alliance Health has developed a network of service providers that deliver a wide range of innovative community-based services to individuals with severe and persistent mental illnesses, substance use disorders, and intellectual and developmental disabilities. As a fully integrated BH I/DD Tailored Plan, Alliance Health contracts with numerous provider organizations and systems that have expanded their network to include primary care providers, specialists, ancillary health providers, and pharmacies, providing a more comprehensive approach to members’ care. Alliance Health also provides care management services to all BH I/DD Tailored Plan members, either directly or through contracts with certified provider organizations.
Alliance Health has partnered with CityBlock to provide eligible members with access to CityBlock’s primary care and behavioral health services. Members can meet with their care team virtually, in their homes or at an existing CityBlock clinic. CityBlock’s Care Model It connects enrolled, eligible members to a multidisciplinary care team that can include primary care providers, behavioral health specialists, community health partners, nurse care managers, pharmacy navigators and more.
Alliance Health and Cityblock have entered into a value-based arrangement – Alliance Health’s first risk-sharing arrangement – to establish a strong alignment towards a shared goal of providing quality health care and outcomes to members. This care structure gives care teams the flexibility to intervene and invest in dynamic strategies to meet members’ complex clinical and non-clinical needs. Alliance Health and Cityblock’s value-based partnership will improve access to uniquely integrated care for our most vulnerable members, help close health equity gaps and drive sustainable value.
