Technology-enabled infusion platform Uptiv Health has announced a partnership with Lyn Health, a multi-chronic care provider, to combine infusion therapy and chronic care management through a service called Uptiv360.
Through this partnership, patients will be able to undergo an onboarding process via an app and then be evaluated by a nurse during their infusion visit. Within 48 hours, patients will be connected to their Lyn Health care team and have access to virtual chronic care managed care, including behavioral health services.

Through Uptiv, patients have access to a multidisciplinary care team, including access to care coordination, care navigation, behavioral health and medication management services.

Uptiv launched in August last year with $7.5 million in funding.
In December, the company introduced a patient engagement application that allows patients to communicate with clinical teams through chat and video sessions, manage appointments, perform digital check-in and onboarding, and access medical histories.
Through Uptiv’s app, patients also have access to community and caregiver services, including billing and payment services, care planning and coordination, and adding a caregiver to their profile.
Another infusion company leveraging technology is Local Infusion, which raised $10 million in Series A funding in November.
